NO MEDICATION WILL BE ADMINISTERED EXCEPT UNDER THE FOLLOWING CIRCUMSTANCES: Only prescription medication accepted. Must have pharmacy label on medication container. Must have a Form # 135 filled in by both physician and parent copy (copy in back of handbook).
